What a Hospitalist Means to Patients	Share \|

Cogent’s highly qualified, experienced physicians, known as hospitalists, specialize in caring for patients while they are in the hospital. Hospitalists work in partnership with your referring physicians and are supported by nurse Clinical Care Coordinators (CCCs). Cogent Hospitalists are experts of inpatient hospital medicine. These physicians care for a hospital’s Primary Care Physicians’ (PCPs) patients when they visit the emergency room or are admitted to the hospital. Hospitalists serve as specialized physicians, delivering a quality of care otherwise not readily available to inpatients in place of seeing their PCP. All patient information is continually relayed to PCPs at admission, during the hospital stay and at discharge.
